Here's my lame tweakage guide:

Drivers:

Make sure you have the most current drivers for your Video Card, Sound Card and Motherboard.

ATi users:

Make sure you have the beta 4.9 drivers with the Doom optimizations that you can D/L here:
Herrrrr

Under "OpenGL" in your ATi Driver Settings click "compatibility" and set "Triple Buffering" to "Enabled".

Make sure you have AA and AF turned off.

These tweaks are done to the DoomConfig.cfg file that can be found in the base directory of your Doom3 folder:

seta s_maxSoundsPerShader "1"
(This setting is specifically for Audigy2 users. It will correct some choppiness due to sound problems and allow you to hear the in-game music)

seta image_useCache "1"
seta image_cacheMegs "256"
seta image_cacheMinK "20480"
(Crazy amounts of FPS boost and choppiness/pause reduction come out of this change. If you have more than a gig of ram you can try setting the CacheMegs variable even higher. Please note, however, that this improvement was on my dual-channel AMD machine and that changing the useCache setting had detrimental results on Slayer's Intel machine.)

seta com_showFPS "1"
(Enables in-game frame rate display)

seta image_anisotropy "0"
(This one is ATi specific and only really needed to reduce lag under "High" or "Ultra High". It will fuzz up the textures a bit so only use it if you need to.)

You can decompress the PAK files using WINRAR and place the contents in the BASE folder for faster access. This has been reported to FIX MANY studdering and performance problems on many machines.

Ask me how to do this and I will give you details. Note: this takes the HD requirement for Doom close to 4gigs and may not help too much if you are running a real fast drive (SATA, 7500+ etc).

seta image_useCache "1" gives me a pretty bad performance hit.

I changed it back to 0 and its running smooth again. Heres a screenshot of Ultra Mode, although it doesn't look as good as the 3gig BMP since this is a compressed JPG.
